How to Enroll in Hospice - It's an Easy Process!

• The enrollment process begins with a simple phone call to set up an interview with an AmHeart Admissions Coordinator.
  
  
• After the interview, the patient’s individual Plan of Care will be developed and discussed with your family. Our objective is to design the most comprehensive hospice program based on the patient’s specific situation and needs.
  
  
• Once the program starts, the case manager will provide regular updates to the family and the referring physician.
