Deciding When to Initiate Hospice Care

According to national statistics, the average length of stay for patients in a hospice program is about 50 days. The earlier the hospice team intervenes, the more satisfaction is expressed by patients and families. Referring to hospice earlier provides access to the full scope of hospice services available to the patient and family.

We Provide One-On-One Attention

Our nurses, aides and care team members give undivided attention to the patient. They are there to see just one patient, to concentrate on one patient. We maintain a low nurse-to-patient ratio. This individual attention is delivered in your facility alongside your caregivers. The result is a family that is completely satisfied with the extra care and attention their loved one is receiving.

We Make Your Life Easier

We will work with you to jointly develop an individual plan of care for each AmHeart patient in your facility. We adapt our program to your facility. We can use your pharmacy, your physical therapy department. We even comply with the manner in which you chart your patients.

Our RN Patient Care Coordinators and our nurses monitor continually to make certain that all documentation appears in both your charts and files as well as ours.

We Make Billing Easy

Once an item appears on the patient's Plan of Care, we provide it and we pay for it.

We Reimburse More Quickly

We are responsible for our Medicaid patients' monthly room and board, minus the Patient Surplus. Once the patient is Medicaid approved, we typically reimburse within 30 days.

There are occasions when our patients and families at home ask for information about Nursing Homes. As their condition changes or worsens, they need the care and security of a long-term care facility. We are able to assist them in their decision making process.
