AmHeart Services
- Patient education and communication - Clear, honest information allows patients and families to make informed decisions in choosing care.
- Education to prevent "crisis" situation, decrease family anxiety and prevent unnecessary hospitalizations.
- Skilled nursing and aide visits; chaplain, volunteer service, speech, physical or occupational therapy, and dietary counseling, as required.
- AmHeart is Medicare/Medicaid certified and works with private insurers.
- Bereavement support for the whole family.

The hospice patient is cared for by the AmHeart Care Team that is made up of social workers, RNs, LPNs, home health aides, chaplains, physical, occupational and speech therapists, and specially trained volunteers. |
Supportive services to patients and families - physical, social, spiritual and emotional care support for the family that continues through the mourning process.
- Palliative care - care to relieve pain, control symptoms and provide comfort to the patient.
- 24-hour on call support - a hospice RN is on call 24 hours a day, seven days a week, in case of an emergency.
- Physician directed interdisciplinary care - Under the direction of a qualified physician AmHeart will administer medications and wound care.
- Assistance with all Medicare / Medicaid paperwork
